Job Purpose: As a Care Coordinator, you will be responsible for ensuring the delivery of high-quality domiciliary care services within the contracted service area of Witham, Essex, and surrounding villages. You will oversee the day-to-day coordination and preparation of rotas, care packages, and care staff, while providing support and supervision to the Senior Care Assistants. Collaboration with the Review Officer, local authorities, and adherence to CQC regulations are essential in this role. Your contributions will aid in the development of domiciliary care services in Essex to meet the evolving needs of service users and contractual obligations. This position requires participation in the on-call/out of hours duty rota and the maintenance of up-to-date on-call documentation.

Key Responsibilities:

Area Coordinating:

  • Supervise and support the care team in delivering services to the highest possible standard, ensuring all visits are covered and no visits are missed or canceled.
  • Reallocate visits and rounds promptly and safely, ensuring direct communication with care staff and confirming instructions via text to avoid misunderstandings.
  • Train and support Senior Care Assistants in providing the on-call service and completing compliance-related tasks.
  • Ensure daily communication and updates with on-call staff regarding service users, hospital admissions, and general issues.
  • Enter all changes occurring outside office hours onto the roster on the next working morning.
  • Alert the Review Officer regarding required visit reductions/increases/changes to care/support plans or when risk assessments need reviewing.
  • Monitor staff adherence to policies, procedures, and processes.
  • Maintain knowledge of relevant legislation, CQC standards, best practices, and codes of conduct.
  • Ensure timely delivery of new or updated Support Plans, including associated risk assessments.
  • Provide support and care in the community if the Business Continuity Plan is activated.
  • Contribute to a weekly care staff newsletter.
  • Maintain a tidy and professional office environment.
  • Record concise and effective journal notes, highlighting the main points.
  • Ensure accurate recording of care staff sickness, absences, and training attendance on the roster.
  • Provide explanations for frustrated or canceled visits.
  • Maximize continuity of care when assigning rounds to care staff.
  • Handle commissioning enquiries and new package offers, ensuring accurate arrival time windows for visits.
  • Seek authorization from the Office Manager before refusing any packages.
  • Produce holiday rotas well in advance when necessary.

CQC Minimum Standards and ECC Contract Compliance:

  • Ensure compliance with the annual compliance/supervision program and Care Quality Commission's requirements within the specified time frames.
  • Ensure all care staff are up-to-date with mandatory training in Safeguarding, Medication, and Moving & Handling.
  • Provide appropriate support and induction to new care staff during their probationary period and ensure timely sign-off.
  • Alert the Office Manager immediately regarding care or staff conduct/performance concerns, issues, or complaints.
  • Maintain high standards in service delivery and promptly report any service delivery issues or concerns.

Service Delivery:

  • Manage day-to-day visit changes, ensuring uninterrupted service delivery to all service users.
  • Respond effectively and promptly to service users' queries.
  • Notify service users or their representatives of any operational issues that may affect service delivery, including delays exceeding 30 minutes.
  • Maintain the company's quality assurance procedures.
  • Comply with the Safeguarding of Adults procedures and promptly report any potential safeguarding issues.
  • Perform care rounds when capacity is challenged and work cannot be covered by existing resources.

Relationship/Staff Management:

  • Cultivate positive working relationships with social care and health professionals, as well as other agencies involved in community care provision.
  • Promote equality and diversity in staff management and service delivery.
  • Report all care staff injuries/accidents to the Head of Administration and conduct necessary risk assessments.
  • Conduct immediate risk assessments for carers reporting injuries occurring in service users' homes.
  • Provide immediate risk assessment for care staff reporting pregnancy.
