- Provide counselling and emotional support to clients facing various challenges
- Liaise with other professionals, such as healthcare providers and social services, to coordinate comprehensive care
- Advocate for clients' rights and access to services
- Monitor and evaluate clients' progress towards their goals
- Maintain accurate and up-to-date records of client interactions
