Job Role:

  • Assess, plan, provide, document and evaluate individualised and high quality care which respects the patients’ privacy, dignity and religious and cultural beliefs.
  • Maintain and develop knowledge, skills and competence to ensure patients individual needs are met in accordance with the NMC Code of Professional Conduct, PREP and Trust Policies and Procedures.
  • Participate in the development of quality initiatives.
  • Maintain and provide accurate records, reports and information ensuring that patient and clinical confidentiality is maintained at all times.
  • Contribute to a learning environment which is conducive to the acquisition of education and training.
  • Act as a mentor to students and newly qualified staff, upholding the philosophy of evidence based practice.
  • Maintain and promote a safe working environment for patients, visitors and staff, with particular reference to infection control and health and safety.
  • Maintain and promote effective communication both within the multidisciplinary care team and with other colleagues, patients, relatives and visitors.
  • Assist in the management of the ward budget and ensure appropriate stock levels are maintained.
  • Ensure that repairs and replacements are ordered by appropriate reporting.

Skills Required

  • Registered Nurse on part 1,
  • Evidence of continuing professional development
  • Teaching Certificate – Desirable
  • Current appropriate Life Support Certificate – Desirable
  • Care of the Elderly or other appropriate post registration clinical qualification - Desirable
  • Mentorship qualification – Desirable
  • Clinical competency to assess, provide, maintain and co-ordinate high quality sub-acute care to elderly/rehabilitation patients.
  • Core skills in cannulation, venepuncture, ECGs Evidence of discharge planning.
  • Understanding of the quality issues and auditing.
  • Understanding of confidentiality and implications of noncompliance.
  • Knowledge of health and safety issues within own work area.
  • Ability to make clinical decisions and problem solve.
  • Understanding of clinical governance framework and risk management and implications for practice. Experience of mentoring and preceptorship.
  • Ability to supervise staff.
  • Understanding of NMC standards and implications for practice.
  • Enthusiasm for elderly/rehabilitation specialty.
  • Willing and able to work shifts, including night duty, weekends and Bank Holidays.
  • Willing to work across all Community Hospitals sites to enhance personal and professional development if required.
